Behandeling resectabel oesofaguscarcinoom

Indien locally advanced oesofaguscarcinoom cT1N1M0 of cT2-4aN0-3M0:  

• Neoadjuvante chemoradiatie volgens CROSS (van Hagen, NEJM 2012):
  • Paclitaxel - carboplatin:
    • Paclitaxel 50 mg/m² i.v., dag 1, 8, 15, 22, 29
    • Carboplatin AUC 2 i.v., dag 1, 8, 15, 22, 29
  • Radiotherapie:
    • Totaal 41,4 Gy (23 fracties * 1,8 Gy)
    • 5 fracties per week, starten op dag 1 van chemotherapie
  • Verricht 6 weken na afronding CROSS een PET-CT-scan en bespreek patiënt in MDO SMACON

Behandeling irresectabel oesofaguscarcinoom

Indicaties:

• Irresectabele tumoren (T4b; locatie: hoog cervicaal)
• Niet operabele patiënt (veel co-morbiditeit)

Definitieve chemoradiatie volgens CROSS maar dan 6 i.p.v. 5 weken:

• Paclitaxel - carboplatin:
  • Paclitaxel 50 mg/m² i.v., dag 1, 8, 15, 22, 29, 36
  • Carboplatin AUC 2 i.v., dag 1, 8, 15, 22, 29, 36
• Radiotherapie:
  • Totaal 50,4 Gy (28 fracties * 1,8 Gy)
  • 5 fracties per week, starten op dag 1 van chemotherapie
